Before the visit kindly print out and complete the “Circumcision Information Package (.pdf)” which consists of a registration form and consent form.

Bring the 2 completed forms with when your son comes for his circumcision. (Consult and surgery done at same visit)

Recommend:

• feeding your son before leaving home.
• Tylenol 1 ml. before leaving home. 
• bring along extra diapers.
